Application Checklist
Checklist to complete the application process:
- 4 Passport Size Photos: Child
- 1 Copy of Child’s Birth Certificate & MyKad /Passport
- 1 Passport Size Photo of each parent
- 1 Copy of Both Parents’ Passport & Visa (for non-Malaysian)
- 1 Copy of Recent School/Academic Reports
- Medical Report/Vaccination Record
- Proof of Guardianship if necessary
